• Reimbursement Claim Document
  This form is used to request a reimbursement of medical/surgical health expenses, previously paid by the insured person. This form should be completed and sent to the contacts listed in it, along with original receipts of expenses.

• Accident description form
  This form should be used to communicate the circumstances in which an accident occurred. If our services request, fill in the marked field in the document and send it to the contacts listed in it.

• Access form for the International Networks
  For access to international networks it is necessary to fill out this form so we can review your request and issue the required authorizations. This form should be completed and sent to the contacts listed in it.
